HealthMEDX Telemedicine Partnership Focused On Non-Acute Provider Benefits

New telemedicine company Connected for Care debuts its product line at this week’s annual LeadingAge annual conference. Formed by senior care executives looking to extend their technology benefits, the company has partnered with leading non-acute EMR software developer HealthMEDX to more closely connect providers with patients and further reduce hospital readmissions

Together with strategic software partner HealthMEDX®, Connected for Care will demonstrate the benefits of pairing telemedicine tools with a comprehensive EMR platform in non-acute care settings.

Connected for Care’s debut follows a 10-month pilot with Lexington Health Network, a leading post-acute provider in the Chicagoland area. During the project, Lexington’s care providers utilized a prototype CARE Telemedicine solution to remotely evaluate sudden acute changes in condition for more than 250 seniors in Lexington’s LTPAC facilities. Of that group, three out of four patients were able to be treated in place and thus avoided a return hospital visit, saving the Health System an estimated $2.5 million in transfer and direct care costs. Even beyond monetary savings, the readmission measure is a key quality metric and business differentiator in Lexington’s market, where providers have been penalized for avoidable hospital readmissions at 20 percent higher frequency than the national average.

Driven by the Readmission Reductions Program and IMPACT Act, the Connected for Care founders sought a way to extend the clinical quality improvements achieved by Lexington through enterprise EMR implementation and usage. The resulting solution is a custom telemedicine cart, complete with secure HD video, a digital stethoscope, touchscreen control center and full chart access via the HealthMEDX® iCare mobile app. The leased units allows bedside nursing staff at long-term and post-acute care organizations to facilitate direct physician-patient assessments, while concurrently reducing provider travel, encouraging remote rounding and elevating collaborative care.
